Illinois Association of Community Care Program Homecare Providers (IACCPHP) Home Page

Purpose: Information and advocacy for in-home care services to Illinois elderly served though the Illinois Department of Aging (IDoA) Community Care Program (CCP).

Who We Are: The Illinois Association of Community Care Homecare Providers (IACCPHP) is an association representing the approximately 60 agencies contracted with the Illinois Department on Aging (IDoA)to provide in-home assistance to Illinois’ elderly with activities of daily living through the Community Care Program. Approximately 40,000 clients are served through the program. Our members employ over 18,000 homecare assistants.

IACCPHP, formed in 1988, is comprised of large and small providers, as well as both not-for-profit and for-profit providers. Association members work in conjunction with IDoA and other aging network associations to improve CCP services and to enhance other community based services for seniors.

In-home Services: These services provide assistance with household tasks including; cleaning, laundry, shopping, errands, and planning and preparing meals. Homecare Assistants also assist clients with personal care tasks such as dressing, bathing, and grooming.


What is the Community Care Program? The Community Care Program (CCP) is a state program created in 1979 by Public Act 81-202 for the prevention of premature and unnecessary institutionalization of seniors by providing in-home and community based services. CCP is managed by the Illinois Department on Aging (IDoA) and is recognized as a national model of in-home and community based care to at-risk seniors. In Fiscal Year (FY) 2002, the program served approximately 40,000 frail older adults with overall expenditures approximating $220 million.

Funding for CCP comes through the State General Revenue funds as well as through a Federal Medicaid Waiver and in some cases a client co-pay portion exists. The Program saves state tax dollars each year through the avoidance of unnecessary and premature placement of clients into nursing homes or other more costly alternatives.

CCP Eligibility:
To receive CCP services, an individual must be at least 60, a U.S. citizen or legal alien, a resident of Illinois, have nonexempt assets of $12,500 or less (nonexempt assets do not include home, car or personal effects or household furnishings) and have an assessed need based on a physical and mental determination for long term care.

Assessment of need is made by independent Case Coordination Units (CCU’s). For additional information about eligibility or to request an assessment call Illinois’ Senior Help Line at
800-252-8966. If approved for CCP services by a CCU, a Medicaid application is also required.
